Direct Patient Care

 

DPC memberships cover all physician care and interactions by ePhysician Services Providers, either virtually or in person. The monthly cost depends upon your family unit and primary member’s age. 

 

 

Lab testing and Imaging can be obtained at discounted rates through ePhysician Servicces, but are not covered in the monthly subscription rate. 

 

IF YOU ARE MEDICARE AGE (62+), please note that due to federal law we are not permitted to provide services to anyone who is registered with Medicare. This is not our choice, but government imposed, and carries with it serious penalties. If you continue services with us after age 62, the premium is $99/month per individual over the age of 62, and the policy must be immediately cancelled in the event that the individual signs up for, or receives without being aware of signing up, Social Security or Medicare.

 

Counseling or Coaching Services

 

General Counseling Services for Individuals are billed at $175 for a 50 minute appointment. All these appoinments will be virtual and completely confidential.
